We want to thank you for choosing Davis Eye Care. We know that you have many choices for your healthcare and we are glad you’ve chosen us. The purpose of this page is to help inform new patients about the policies and procedures of our office.

Financial / Insurance

We accept most major health insurance plans, including those listed below. If you do not see your plan on this list, please contact our office to confirm our participation. This list is subject to change without notice. Please always call our office to confirm our participation. If we accept your insurance, we will file a claim with your insurance company and you will be responsible for any co-pays, deductibles or non-covered services. For patients without insurance or insurance we are not contracted with, full payment will be due on the day of treatment. For your convenience, we accept cash, debit and credit cards.

At Davis Eye Care, we see patients by appointment. We request that you give our office 24 hours notice if you are not able to make your scheduled appointment.

If you have an emergency, we will always do our best to work you in to be seen the same day.

Prescription Refills

Prescription refill requests should be made at least 2 business days prior to the desired pick-up date to allow us time to process your prescription. When you call our office, please have the following information available: pharmacy name, pharmacy phone number, name of prescription to be refilled and dosage.

We do not refill prescriptions after hours.
